I understand that submitting this booking request form does not guarantee an appointment.

I acknowledge that I will be contacted via email for follow-up, and I will be required to submit a deposit within 24 hours of confirming my appointment with the artist.

I acknowledge that my appointment will not be confirmed until the deposit is received. I understand that the deposit is nonrefundable.

If I need to reschedule, I will do so at least 72 hours before the appointment to avoid forfeiting my deposit.

I understand that if I make changes to my design after submitting this request, the cost may differ from the original quote provided.
